      Carrier screening offers meaningful benefits but also carries limitations that are important for patients to understand before testing.
      Benefits Include the opportunity to learn about reproductive risk before or early in a pregnancy, time to consider the full range of reproductive options, the ability to plan for specialized prenatal or newborn care if a pregnancy is found to be affected, and reduced anxiety for the large majority of people who are found not to carry changes in the genes tested.
      Limitations Include the fact that no panel tests for every possible genetic condition, meaning a negative result does not guarantee an unaffected child; the existence of residual risk even for genes that are tested, since not every possible pathogenic variant is detectable by current technology; and the possibility of complex or uncertain results, such as a variant of uncertain significance, where it is not yet clear whether a particular genetic change is disease-causing.
      Panel size and content Vary between laboratories; while many laboratories follow the standardized Tier 3 panel, others offer larger panels including additional, rarer genes, referred to as Tier 4, or smaller panels. Discussing exactly which conditions a specific panel covers is a useful part of pre-test counseling.
      Psychosocial and ethical considerations Also arise, since carrier screening results can affect family planning decisions, relationships between partners, and, for some conditions, raise complex questions about disability and quality of life that different individuals and communities may view differently.
      Pre-test and post-test counseling Recommended for all patients undergoing carrier screening, ideally with a genetic counselor or other qualified professional, to ensure that patients understand what is being tested, what results might mean, and what options exist depending on the outcome.

      Because the field of carrier screening continues to evolve, with panel content, technology, and professional society recommendations updated periodically, patients are encouraged to discuss the most current recommendations with their own care team at the time testing is considered.
