What Are Soft Markers?
Soft markers are minor variations seen on a fetal ultrasound that are not structural abnormalities but are statistically associated with a slightly higher chance of chromosomal conditions (such as Down syndrome/Trisomy 21, Trisomy 18, or Trisomy 13). They are not defects — they are subtle sonographic findings that can occur in completely healthy fetuses as well.
Common soft markers include:
Nuchal fold thickening
Echogenic intracardiac focus (EIF)
Echogenic bowel
Mild pyelectasis (renal pelvis dilation)
Shortened femur or humerus length
Choroid plexus cysts
Absent or hypoplastic nasal bone
Absent or hypoplastic retronasal triangle (discussed below)
Soft markers shift risk, they don’t confirm or rule out a condition on their own. A single isolated soft marker in a low-risk pregnancy is usually not significant. Multiple markers, or a marker combined with an already elevated risk (from maternal age, NIPT, or biochemical screening), carries more weight.
Soft marker findings are used to refine existing risk assessments. They often prompt a conversation about further testing (NIPT, amniocentesis) depending on background risk. It usually leads to a follow-up scan, a genetic counseling discussion, and sometimes a recommendation for NIPT or diagnostic testing.
Limitations: Soft markers have low specificity, many normal fetuses show one marker. Their significance depends heavily on gestational age, whether it’s isolated or multiple, and the mother’s baseline risk. They are not diagnostic; they are probabilistic.
