Male infertility is broadly classified based on where the problem originates:
*Pre-testicular causes: Hormonal imbalances originating outside the testes, such as low levels of gonadotropin-releasing hormone, luteinizing hormone, or follicle-stimulating hormone, which disrupt the signals needed for sperm production. Conditions like hypogonadotropic hypogonadism fall here.
*Testicular causes: Direct problems within the testes affecting sperm production or quality. This includes varicocele (enlarged veins in the scrotum), undescended testes, testicular injury or infection, chromosomal conditions such as Klinefelter syndrome, and certain genetic mutations affecting genes like CFTR or Y-chromosome microdeletions.
*Post-testicular causes: Sperm are produced normally but cannot be delivered effectively. This includes blockages in the vas deferens or epididymis, absence of the vas deferens (often linked to CFTR gene mutations), retrograde ejaculation, and erectile or ejaculatory dysfunction.
*Idiopathic male infertility: No identifiable cause is found despite thorough evaluation, which accounts for a notable proportion of cases and may involve subtle genetic, epigenetic, or environmental factors not yet fully understood.
