      Amniocentesis is considered a safe procedure overall, and the risks, while real, are small. It is natural and completely reasonable for a family to want to understand these risks clearly before deciding whether to proceed.

      Pregnancy loss is the risk that concerns most patients, and it is also the one that has been studied the most. Current guidance from the American College of Obstetricians and Gynecologists estimates the chance of miscarriage caused directly by the procedure at approximately 1-in-900, a considerably lower figure than the roughly 1-in-200 estimate that was quoted for many years. This shift reflects improvements in ultrasound guidance, needle technique, and operator experience, along with larger and more rigorous research studies. Most procedure-related losses that do occur happen within the first several days after the procedure. For comparison, chorionic villus sampling, the earlier alternative procedure using placental tissue, carries a slightly higher estimated loss risk of approximately 1-in-455.

      Leakage of amniotic fluid or light bleeding can occur at the needle insertion site and usually resolves on its own without treatment or harm to the pregnancy.

      Infection is rare, since sterile technique is used throughout the procedure, but it remains a small possible risk of any procedure that enters the uterus.

      Transmission of certain infections from parent to baby, including hepatitis B, hepatitis C, and human immunodeficiency virus, is a specific concern for patients who carry these infections, since the needle passes through infected tissue on its way to the amniotic fluid. This risk can often be reduced with appropriate medical management beforehand, so patients with these conditions should discuss individualized precautions with their care team.

      Needle injury to the baby is exceedingly rare given that the procedure is performed under continuous ultrasound guidance.

      Every family’s individual risk can vary somewhat depending on factors such as the experience of the person performing the procedure, whether the pregnancy involves twins or more, and the timing of the procedure. A genetic counselor or maternal-fetal medicine specialist can help place these numbers in context for a specific pregnancy and can also discuss chorionic villus sampling or continued screening as alternatives.

      It can help to weigh the small added risk from the procedure itself against the reason testing is being considered in the first place. Every pregnancy carries some background chance of miscarriage that has nothing to do with amniocentesis, and that background chance, particularly earlier in pregnancy or in a pregnancy already flagged by a screening test or ultrasound finding, is generally larger than the roughly 1-in-900 risk the procedure itself adds. Likewise, when amniocentesis is being considered because of a higher-risk screening result, the chance that the pregnancy is actually affected is often meaningfully higher than the chance of a procedure-related loss. Framed this way, the small risk added by the procedure is usually modest compared with the uncertainty a family is already living with, which is one reason many patients ultimately choose to move forward with testing. This is, of course, a personal decision, and a genetic counselor can walk through the specific numbers that apply to an individual pregnancy.
