KeymasterSilent carriers and people with alpha-thalassemia trait do not need treatment. The main practical point is to avoid iron tablets unless iron deficiency has actually been confirmed by blood tests.
For hemoglobin H disease, many individuals need little treatment, but good everyday care prevents problems:
Folic acid A daily folic acid supplement supports the bone marrow, which works harder than usual to replace red blood cells that break down early.
Avoiding triggers Certain medicines and substances can damage red blood cells and cause a sudden drop in hemoglobin. The list is similar to the one given for glucose-6-phosphate dehydrogenase deficiency, and includes some antibiotics, antimalarial medicines, and mothballs. Always tell doctors and pharmacists about the diagnosis before starting new medicines.
Prompt care for fever and infection Fever is the most common trigger for a hemolytic crisis. Many crises need close monitoring, and some need a blood transfusion.
Blood transfusions Most people with deletion-type hemoglobin H disease only need transfusions during crises, surgery, or pregnancy. Some with more severe forms need regular transfusions.
Iron chelation therapy Even without transfusions, the body absorbs more iron from food in hemoglobin H disease, and iron slowly builds up in the liver and other organs. Iron-removing medicines are started when blood or liver tests show the iron level has become too high.
Removal of the spleen This may be considered when anemia is severe or crises are frequent. It raises the risk of infection and blood clots, so it is weighed carefully, and blood-thinning precautions are used afterwards.
Mitapivat In December 2025, the United States Food and Drug Administration approved this oral medicine for anemia in adults with alpha-thalassemia or beta-thalassemia. It helps red blood cells produce energy and survive longer. It needs regular liver monitoring because of a risk of liver injury, and its availability in India is still limited. For hemoglobin Bart’s hydrops fetalis, treatment may begin during pregnancy. Blood transfusions given to the baby in the womb, called intrauterine transfusions, can reverse the fluid build-up and allow some babies to survive. After birth, these children need regular transfusions and iron chelation for life, unless they have a hematopoietic stem cell transplant. This transplant replaces the bone marrow with healthy donor stem cells and is currently the only cure. The decision to continue a pregnancy with intrauterine treatment is personal and deserves detailed discussion with a fetal medicine specialist and a genetic counselor, since the baby’s long-term health and the mother’s own health are both involved.
